Nurse Practitioner
- Palliative Care

 

Hospice Eastern Bay of Plenty (Hospice EBOP) is a not-for-profit organisation who provides local community based palliative care for patients and their family/whanau living with life limiting conditions.

Our mission is to provide excellent, compassionate palliative care for everyone throughout the Eastern Bay of Plenty and we are seeking an experienced Nurse Practitioner to join our team, to help achieve this goal.

At Hospice EBOP we work as a team to ensure we provide the care needed to allow our patients to live well when their time is limited, but also do our part individually to ensure we can successfully achieve our objective.

We have a skilled team who are committed to delivering optimum care for both patients and their families / whānau. This Nurse Practitioner role further extends our reach by providing advanced clinical expertise and leadership across community settings.  Registered Nurse
 

Hospice Eastern Bay of Plenty (Hospice EBOP) is a not-for-profit organisation who provides local community based palliative care for patients and their family/whanau living with life limited conditions.

Our mission is to provide excellent, compassionate palliative care for everyone throughout the Eastern Bay of Plenty and we are seeking an experienced Registered Nurse to join our wonderful team, to help achieve this goal.

 

At Hospice EBOP we work as a team to ensure we provide the care needed to allow our patients to live well when their time is limited, but also do our part individually to ensure we can successfully achieve our objective.

 

ABOUT THE ROLE

Managing and coordinating the palliative care of up to 25 community patients in the Eastern Bay of Plenty, whilst supporting the Clinical Services Manager. Making daily calls to patients and visiting when necessary to those with complex care requirements. Liaising with patient’s GP practices, Allied Heath, carers, family/whanau and colleagues to ensure the best care is provided to each patient.

 

WE ARE LOOKING FOR SOMEONE WHO:

  • Has an interest in palliative care

  • Excellent communication and patient assessment skills

  • Shows compassion, respect and integrity

  • Ability to work independently and enjoy being part of a supportive and collaborative team

  • Strives for excellence in all aspects of their role

  • Current NZ Annual Practising Certificate

  • Applicants should have NZ residency or a valid NZ work visa.

**Study leave support can be made available for the right candidate to complete palliative care qualifications.

This is a permanent full time position.

Clinical Nurse Specialist

Our mission is to provide excellent, compassionate palliative care for everyone throughout the Eastern Bay of Plenty and we are seeking an experienced Clinical Nurse Specialist to join our team, to help achieve this goal.

At Hospice EBOP we work as a team to ensure we provide the care needed to allow our patients to live well when their time is limited, but also do our part individually to ensure we can successfully achieve our objective.

We are seeking an experienced Palliative Care CNS with a passion for the provision of integrated community palliative care and education.

  • To provide an in-reach / liaison service to Public Hospital (PH) and Aged Residential Care (ARC) facilities to support the delivery of best practice palliative care for Hospice EBOP patients 

  • To provide ongoing clinical care / support to HEBOP patients in ARC facilities

  • To coordinate and deliver Hospice New Zealand (HNZ) Fundamentals of Palliative Care Program and HNZ Syringe Driver Competency Program to ARC facilities and generalist palliative care providers

  • To provide coaching and mentoring to staff in ARC facilities across the region to support the delivery of quality palliative care to residents and their families /whanau

  • To promote and encourage a learning environment within Hospice EBOP clinical team, facilitating and coordinating responsive and comprehensive learning opportunities

 

We would love to hear from you if you have:

  • 5+ years’ experience in, and sound knowledge of, specialist palliative care

  • Relevant post graduate qualifications in palliative care (minimum: PG cert, working on PG diploma)

  • Advanced patient assessment skills

  • Advanced communication skills

  • Ability to work independently in the community

  • Passion for education delivery
